The in-reach re-ablement service is a physiotherapy-led step-down service that allows patients to be discharged from hospital into a care home while having their social care needs assessed.
Following a hospital stay, some patients may struggle to return home until they have received a course of re-ablement.
Patients may require a package of care with up to four visits a day which can be difficult to source and costly to provide.
If patients don’t have the right type of support or equipment at home, hospitals may be unable to discharge them, and they remain in a hospital bed.
HomeLink Healthcare’s in-reach re-ablement service supports patients in care homes with short-term physiotherapy and re-ablement. The aim is to improve mobility and avoid further deconditioning in preparation for returning home safely within a few weeks.
Whilst in hospitals or care home environments, patients may become deconditioned, leading to them to lose confidence, muscle strength and endurance. These factors combined can lead to an increased risk of falls, poor mobility and increased care needs. By receiving timely regular support that is tailored to individual needs, patients are able to recover and return home more quickly.
Tracking clinical outcomes (EQ-5D-5L) and mBarthel scores enable us to evidence the progression of our patients.
