HomeLink Healthcare is a clinician-led, specialist Hospital at Home service provider. We have been delivering safe, high-quality services to NHS patients in the place they call home since 2016.
By working in partnership with hospitals, ICBs, consultants, local authorities, community providers and GPs to provide ‘out of hospital’ care we prevent some people being admitted to hospital and enable others to come home more quickly.
We provide Hospital at Home services through a number of pathways: Early Supported Discharge, Discharge to Assess, Virtual Wards, Intermediate Care at Home, Re-ablement, Rehabilitation, Admission Avoidance, and Bridging Packages of Care.
HomeLink Healthcare is a named supplier on the NHS SBS Patient Discharge Services Framework Agreement. By using the framework, you can contract with us directly and we can get a new service up and running in around 12 weeks.

Benefits of Hospital at Home services
Hospital at Home services free up hospital capacity, improve patient flow, reduce delayed discharges, deliver great patient outcomes and cost significantly less than in-hospital care.
According to NHS England:
- Patients are 5x less likely to acquire an infection on a virtual ward compared to an acute setting.
- Patients are 8x less likely to experience functional decline on a virtual ward compared to an acute setting.
- 2.5x fewer patients treated on a virtual ward are re-admitted to frailty beds than the national acute benchmark.
- 23% of patients achieve a more independent social care outcome when treated in a virtual ward than they would have in an acute setting.
